All registration forms require the latest version of Adobe Reader to view. You can download Adobe Reader for free by clicking here.
To fill out the form, you will have to download the form first by right clicking on the link and then click on “Save Link as…” and choose where you want to save it. Then open it using Adobe Reader. After you have entered all your information you can then save the PDF form and email it to saint.michaels@cttcs.ca or you can print it out and drop it off at the school.
If you have Google Chrome, and if you have installed the Adobe Reader plugin in Google Chrome, you can open and save that way.
* All Registrations must fill out the permission for use of student work and personal information form.
